| dc.description.sponsorship | Introduction: Parenteral nutrition (PN) in premature infants is an important strategy to prevent nutritional deficits during the early stages of life. However, the evidence about the optimal time to start it, before or after 48 hours, continues to be heterogeneous. Objective: To compare the effects of early and late onset of NP on growth, metabolic and hepatic complications, duration of hospitalization and infections, in addition to evaluating the methodological quality of the available studies. Method: A total of 23 studies were included in prematures <37 weeks that compared PN started before and after 48 hours. The searches were conducted in PubMed, Elsevier, BMJ, MDPI, Sage Journals, Springer and BMC, with updates until July 2025. The risk of bias was assessed using the NIH, RoB 2 and ROBINS-I tools. Results: The PN started before 48 hours was associated with greater weight gain, better weight recovery at birth and better growth z-scores. It was also shown to have a higher risk of hyperglycemia and biochemical alterations, but a lower incidence of cholestasis. The duration of PN was shorter and a tendency to reduce hospital stay was observed, while nosocomial infections showed variable results. The methodological quality was intermediate, with a predominance of moderate bias risk and heterogeneity between studies. Conclusion: Early PN showed benefits in growth and possible reduction of hospitalization, although it requires surveillance of initial metabolic alterations.
