    Inteligencia artificial en la detección de caries dental en radiografías periapicales.

    Artificial intelligence (AI) has been advancing in dentistry, especially when detecting dental caries. Currently, a series of studies have indicated that AI models, particularly convolutional neural networks (CNN), are ahead of dental professionals. This work aims to determine the differences in the detection of dental caries by analyzing periapical radiographs between traditional methods and artificial intelligence in terms of accuracy, sensitivity, and specificity. This study was conducted through a systematic literature review with scientific databases, including Web of Science, PubMed, Cochrane Library, and IEEE Xplore. The results indicate that convolutional neural networks can significantly improve caries detection. However, there is a need for further research to establish whether it is the definitive tool for caries diagnosis. In conclusion, while artificial intelligence represents remarkable progress in dentistry, its widespread implementation requires addressing impediments, such as performance variability depending on the data type and images. Nevertheless, its ability to improve diagnostic accuracy and its relevance to clinical practice makes it a promising tool for advancing dental radiographic diagnosis.     Nivel de ansiedad de acuerdo a la escala de corah modificada en los pacientes que asisten a la consulta odontológica en la Universidad Católica de Cuenca sede Azogues periodo 2024-2024

    OBJECTIVE: This study aimed to determine the anxiety level in adult patients attending dental consultations at the Catholic University of Cuenca, Azogues Campus, using the Modified Corah Scale from March to August 2024. MATERIALS AND METHODS: This was a descriptive, observational, and retrospective cross-sectional study in which 200 surveys were conducted among patients attending the university clinic from the fifth to tenth academic cycle, assessed with the Modified Corah Scale. RESULTS: According to the survey results, most patients experienced moderate anxiety (66 patients) or high anxiety (63 patients). A significant group presented extreme anxiety or phobia (41 patients), indicating intense fear or aversion to dental treatments. Only 30 patients reported mild or no anxiety. CONCLUSION: According to the Modified Corah Dental Anxiety Scale, the prevailing level of anxiety is moderate, indicating that more than half of the patients experience a considerable level of apprehension toward dental consultations. Understanding each patient’s anxiety level is crucial as it can improve care quality and facilitate the performance of various treatments. Keywords: dental anxiety, dental care, modified Corah scale

    Consecuencias en cavidad bucal producto de la radioterapia en el cáncer de cabeza y cuello. Revisión bibliográfica

    Radiotherapy is one of the primary treatments for head and neck cancer, but it results in various complications within the oral cavity. This article aims to describe the oral cavity consequences that occur in the oral cavity when a patient undergoes radiotherapy in a specific head or neck region. The research was conducted as a literature review using scientific databases Scopus, Web of Science, and PubMed. Results indicate that the predominant complications caused by radiotherapy are mucositis, xerostomia, osteoradionecrosis, radiation-induced dental caries, and taste alterations. These issues are intrinsically related to the administered radiation dose and the specific anatomical region irradiated, negatively impacting patients’ quality of life. Preventive interventions, including fluoride application and laser therapy, are essential to mitigate these adverse effects. The study concluded that radiotherapy in individuals diagnosed with head and neck cancer produces significant adverse effects in the oral cavity. Therefore, preventive and therapeutic dental treatment are vital for alleviating complications and improving quality of life, making a multidisciplinary approach necessary for effectively managing these sequelae.     Trastornos musculoesqueléticos y síndrome de Burnout en estudiantes de la carrera de Odontología de la Universidad Católica de Cuenca sede Azogues durante Marzo-Agosto 2024

    Dentistry students are exposed to improper postures, leading to musculoskeletal injuries that can result in loss of stability and body balance. These issues are exacerbated by prolonged postures, excessive force, repetitive movements, and an inadequate working environment. Additionally, both students and professionals in the dental field face challenges to their mental health due to the stress and demands of their work, which can lead to the development of Burnout Syndrome (BOS). Objective: To identify the musculoskeletal disorders and BOS associated with clinical practice in the dentistry students Program at the Catholic University of Cuenca, Campus Azogues, from March to August 2024. Methods: This observational, descriptive, and cross-sectional study involved the administration of the Nordic Kuorinka and Maslach Burnout Inventory (MBI) questionnaires to 198 students, following informed consent and data analysis using IBM SPSS Statistics v.27. Results: There was a prevalence of musculoskeletal disorders in the lumbar, dorsal, neck, and wrist regions. Regarding BOS participants demonstrated low levels of depersonalization, personal accomplishment, and emotional exhaustion. Conclusions: This study confirms a significant prevalence of BOS and musculoskeletal disorders among dentistry students at the Catholic University of Cuenca Campus Azogues.     Dental caries is a common chronic disease in childhood, often associated with poor oral hygiene. Pediatric dentistry plays a vital role in primary prevention, promoting good hygiene habits from an early age. This study aims to develop a dental care guide based on scientific evidence for primary prevention in pediatric dentistry at the UCACUE Clinic. To this effect, a systematic re- view of the scientific literature was conducted on preventive methods, such as fluoride use, dental sealants, and regular dental visits. International and national guidelines were also analyzed and compared with the clinic's protocols. Interviews with pediatric dentists were also performed to identify common practices and areas for improvement. The goal is to create a structured guide that reduces the incidence of caries in children aged 0 to 12, thereby improving children's oral health and the quality of life of pediatric patients. Implementing this guide will contribute to public health by promoting healthy habits from childhood in the community served by the clinic.     Manejo de conducta en pacientes pediátricos utilizando inteligencia artificial

    This review aims to objectively and impartially investigate and evaluate how AI is integrated into behavioral management in pediatric dentistry, comparing it with traditional techniques. Materials and Methods: Following the PRISMA methodology, searches were conducted in electronic databases such as PubMed, Scopus, Taylor & Francis, and Web of Science. Studies in English and Spanish from 2019 to 2024 were included. The selected references were reviewed to identify additional studies meeting the inclusion criteria. MESH terms, Decs, and free terms were used, along with Boolean operators OR, AND, and NOT. Results: Various authors report that AI is frequently used in pediatric dentistry for precise diagnoses and to assist clinicians, dentists, and pediatric dentists in clinical decision-making and behavior conditioning to improve cooperation during treatment. Conclusion: Artificial intelligence is a suitable tool for managing the behavior of pediatric dental patients, though it has not been fully researched yet. New technologies, such as educational chatbots, virtual reality, augmented reality, and image analysis, help pediatric patients feel more secure and cooperate better during dental visits. These technologies also improve individualized diagnostics and treatments, providing better dental care that is less invasive and stressful.     Dental autotransplantation is an advanced technique that offers a solution for patients with tooth loss resulting from trauma, extensive carious lesions, congenital malformations, or other causes that affect the aesthetics and functionality of the oral cavity. Therefore, it is presented as a conservative alternative, although its clinical application can be challenging. This technique has gained international recognition for its ability to restore masticatory function and aesthetics in specific cases. Detailed planning is essential for the procedure's success, including evaluating the viability of the donor tooth and the recipient site and using advanced technologies such as 3D printing to create precise surgical guides. This research aimed to report a case of dental autotransplantation in a 15-year-old female patient with agenesis of tooth 1.5. The retained dental organ 2.5 was used as the donor with the assistance of a 3D-printed model. After a 5-month follow-up, the tooth remained functional, revascularized, and with pulp vitality, indicating proper progression for this treatment.     Abordaje odontológico integral atraumático en paciente pediátrico en los centros de especialidades odontológicas de la Universidad Católica de Cuenca-campus Azogues. Reporte de caso.

    This clinical case addresses a 6-year-old pediatric patient diagnosed with multiple carious lesions in primary teeth. The patient exhibited negative behavior towards treatment, which led to the use of minimally invasive (MI) techniques such as the Atraumatic Restorative Technique (ART), the Hall Technique with preformed stainless steel crowns, and resin restorations with preformed celluloid crowns. These techniques not only reduced chair time but also improved the patient's clinical experience by minimizing the use of general anesthesia and invasive procedures. The treatment focused on restoring the patient's oral health using high-viscosity glass ionomer as the restorative material known for its fluoride release and protective properties for teeth. Silver diamine fluoride (a cariostatic agent) was also applied to halt the progression of active caries. The results were highly satisfactory, restoring the affected teeth' physical, aesthetic, and functional properties, thereby improv-ing the patient's quality of life and oral health. In this case, MI techniques demonstrated high effectiveness, reducing stress for both the patient and the clinical staff. Keywords: Minimally invasive, ART, pediatric dentistry, fluorides

    Background: In stomatology, the importance of guiding thinking, learning, and attention processes has been highlighted through strategies and activity systems that foster critical thinking and operations and actions that enable the efficient development of dental activities. Objective: To analyze the relationship between teaching methods and evaluation techniques used in dental program education. Methods: The research employed a qualitative, descriptive, and cross-sectional design. The target population comprised 40 teachers from the Dentistry Program at the Catholic University of Cuenca Campus Azogues. Results: Significant insights were revealed regarding the correlation between pedagogical approaches and evaluation techniques in Dentistry. The strong association among teaching methods, resources, strategies, and learning approaches suggests an internal coherence within teaching methodologies. In conclusion, this research's findings underscore the need to improve the coherence and synchronization between teaching methodologies and evaluation techniques. Promoting this coherence could enhance dental education holistically, benefiting the teachers ' practices and students' learning achievements.     Prevalencia de la obstrucción de la vía aérea superior en una población pediátrica que acude a la consulta ortodóntica en la Universidad Católica de Cuenca campus Azogues-2024.

    OBJECTIVE: To identify the prevalence of upper airway obstruction by measuring the nasopharyngeal and oropharyngeal diameter using McNamara analysis on lateral cephalometric radiographs in a pediatric population attending the Catholic University of Cuenca, Campus Azogues. MATERIALS AND METHODS: A descriptive cross-sectional study was conducted, analyzing 40 lateral cephalometric radiographs using the McNamara analysis. RESULTS: According to the analysis of all radiographs of the upper airway, 75% (30) of the patients presented upper airway obstruction, while only 25% (10) had patent airways. Of these, 72.5% (29) of the patients had nasopharyngeal obstruction, and 27.5% (11) had oropharyngeal obstruction. CONCLUSION: Based on the McNamara analysis, the upper airway obstruction index revealed that most of the 40 analyzed lateral cephalometric radiographs presented some degree of upper airway obstruction.     Introduction: This study describes the different strategies used to improve oral health in children with visual disabilities, a group often neglected in dental care. It aims to identify and analyze the methodologies and outcomes of interventions designed to enhance oral health in this specific population. Objective: To describe the strategies used to improve oral health in children with visual disabilities. Materials and Methods: A literature search was conducted from 2019 to 2024, reviewing studies from scientific databases such as SciELO, PubMed, Scopus, Lilacs, Medline, and Google Scholar. Inclusion and exclusion criteria were applied, and articles were selected using Boolean operators OR, AND, and NOT. Results: Twenty-three articles were selected, demonstrating a high prevalence of dental caries and periodontal issues in children with visual disabilities. Some interventions, such as education and adapted tools, showed significant improvements in oral health. Conclusion: Despite the development of various educational strategies and intervention techniques, there remains an urgent need for more integrated and specific approaches for this group. Personalized educational programs and further research are recommended to develop more effective and accessible interventions, thus improving these children's oral quality of life.     Precisión de las impresiones digitales utilizando tecnología TRIOS vs impresión analógica con siliconas de adición en Rehabilitación bucal. Revisión Sistemática

    The success of rehabilitative treatment depends on a fundamental process, impressions, which, with technological advancements, aim to replace conventional methods. This study aimed to compare the accuracy of digital impressions using TRIOS (intraoral scanner brand) technology with analog impressions using additional silicones in oral rehabilitation. The research followed the PRISMA methodology and the PICO strategy, selecting recent articles published between 2019 and 2024 in Scopus, PubMed, and Redalyc databases. The results reveal that digital impressions with TRIOS offer superior accuracy, with an average of 30 µm, compared to analog impressions, with an average of 47.67 µm. This difference suggests that digital technology provides greater precision, reducing the risk of clinical complications; however, it entails high investment costs and a prolonged learning curve. The review concludes that both techniques have effective clinical applications, but digital impressions may offer better results in cases that require greater accuracy.     Manejo clínico de fluorosis dental con resina infiltrativa (ICON) en un paciente de 10 años: Reporte de caso.

    This case report presents a 10-year-old male patient who visited the Catholic University of Cuenca, concerned about the appearance of white spots on his teeth. After a thorough medical history and clinical examination, dental fluorosis grade TF2 was diagnosed, according to the Thylstrup and Fejerskov Index. This type of fluorosis, which causes opaque white spots on the dental enamel, was treated with ICON® infiltrative resin. The treatment began with initial prophylaxis using water and pumice, followed by applying 15% hydrochloric acid for two minutes to remove the hypermineralized superficial layer. Then, 95% ethanol was used to dehydrate the enamel and facilitate the penetration of the infiltrative resin. The resin was applied for three minutes, followed by light-curing. Follow-up appointments at one, three, and six months showed significant aesthetic improvements, with better appearance and color stability. The use of ICON® was ideal for masking the white spots associated with fluorosis, enhancing the patient’s self-esteem and quality of life without requiring invasive treatment. This material is highlighted by its ability to infiltrate enamel lesions without removing healthy dental tissue, preserving the tooth structure by penetrating the pores of demineralized lesions.     Abordaje quirúrgico de supernumerario invertido en paciente pediátrico. Reporte de caso.

    Inverted mesiodens represent a clinical challenge due to their abnormal orientation, which can cause short- or medium-term complications. In this case report, the tooth's crown is directed towards the nasal floor, while the root points towards the oral cavity, opposite to normal teeth. This study aimed to present the clinical case of a 7-year-old male pediatric patient referred for the surgical extraction of an inverted intraosseous mesiodens located between the upper central incisors (1.1 and 2.1) and the nasopalatine canal. Radiological and tomographic evaluations identified the inverted position of the supernumerary tooth, which obstructed the normal eruption of the permanent incisor. A surgical approach was performed, which included an anesthetic block, palatal festooned incision, and flap elevation. The mesiodens were exposed through osteotomy, followed by the controlled luxation and avulsion of the tooth. Subsequently, the site was closed with sutures. Postoperative follow-up after 15 days showed favorable healing and partial eruption of incisor 2.1. No complications were related to the nasopalatine nerve or any hemorrhage, highlighting the importance of proper planning and precise surgical execution. This case emphasizes the relevance of early intervention in cases of supernumerary teeth, with proper planning of the pre-surgical, surgical, and post-surgical phases to prevent future complications such as malocclusion, delayed eruption, or dentigerous cysts.     Proper oral health is essential for the general well-being of the population, with a particular focus on the child population. In Latin America, public health faces significant challenges, particularly in providing services to vulnerable communities. Objective: To determine the incidence of poverty on the oral health of the child population in Latin America. Materials and Methods: A systematic search of the topic was conducted using the Population, Intervention, Comparison, and Outcome (PICO) question, using various keywords in the Scopus, PubMed, Web of Science, and SciELO databases to gather relevant information. Second, an analysis of the bias of the studies was conducted using the PRISMA 2020 guidelines for systematic reviews. Finally, a synthesis of the results obtained from the data was presented in an analytical table. Results: The research revealed that poverty is a barrier to oral health, especially for children. Studies show that children living in poverty have higher rates of dental caries and oral health issues due to limited access to dental care and education. There is a need for better public policies to address oral health and reduce inequalities. Conclusion: Poverty is directly related to poor oral health in Latin American children. The poorest children have less access to dental services and care, resulting in more cavities and oral health problems, ultimately affecting their quality of life and development.     Estudio in vitro de la forma y distancia del foramen apical en premolares mandibulares de una población ecuatoriana durante 2024

    Introduction: Endodontic therapy focuses on the correct diagnosis and treatment of pulpal-periapical conditions, where knowledge of apical morphology is crucial for therapeutic success. It ensures the preservation of dental functionality and provides an optimal environment for the healing and repair of periapical tissues, especially in the apical third—an area critical for long-term dental preservation. Objective: To determine the distance of the apical foramen from the anatomical apex and its shape in mandibular premolars. Materials and Methods: An observational and descriptive study was conducted on 300 mandibular premolars. The samples were non-randomly selected and classified as First Mandibular Premolars (FMP) and Second Mandibular Premolars (SMP). The distance of the apical foramen was measured from both buccal and proximal views using a Vernier caliper and a digital microscope, which were also used to assess its shape. Results: The mean distance in FMP was 0.21 mm (buccal view) and 0.19 mm (proximal view). In SMP, the mean was 0.20 mm (buccal) and 0.19 mm (proximal), with no statistical significance. The most common shape of the apical foramen in both groups was round, with 44.74% and 38.64%, respectively. Conclusions: The distance between the anatomical apex and the apical foramen is more consistent in the buccal view. The most prevalent shape is round, facilitating adequate apical sealing in endodontic treatments. Keywords: endodontics, apical foramen, mandibular premolars, root morphology, apical distance

    Durabilidad y estética de carillas cerámicas con técnica de preparación tipo chamfer palatino versus sin preparación: revisión sistemática.

    This research work aims to compare the palatal chamfer preparation technique and the no-preparation technique for the placement of dental veneers, evaluating their durability and esthetics using a systematic review of scientific studies. The methodology applied includes collecting and analyzing articles from databases such as PubMed and Web of Science, focusing on studies comparing these techniques. The results indicate that the palatal chamfer technique offers better marginal adaptation and durability, although it is more invasive. On the other hand, the no-prep technique minimizes tooth wear and has fewer postoperative complications, although it may compromise longevity and esthetics over time. The primary conclusion is that the choice of technique depends on the patient's needs, with the palatal chamfer technique being more suitable for situations with high aesthetic demands and durability.
