Tratamiento quirúrgico ortopédico de paciente con labio y paladar fisurado unilateral. Reporte de caso

Resumen
The prevalence of non-syndromic unilateral cleft lip and palate in Latin America is high, and patients with this anomaly present craniofacial, auditory, psychological, speech and dentition alterations, among others. The objective of this case report is to inform the management and effects of early orthopedic therapy in an infant patient with unilateral cleft lip and palate, the clinical examination revealed scarring of the cleft lip alveolar palate with presence of nasoalveolar fistula on the left side, skeletal class III malocclusion and a negative protrusion. Therapeutically, a Hyrax disjunctor and a Petit facial mask were used for the rapid expansion of the upper jaw, in addition to maxillary protraction. After one year and six months, the transverse expansion of the upper jaw and the protraction of the upper jaw were evidenced, obtaining favorable clinical results thanks to the cooperation of the patient and his parents.

La prevalencia de labio y paladar fisurado unilateral no sindrómico en Latinoamérica es alta, y los pacientes que padecen de esta anomalía presentan alteraciones craneofaceales, auditivos, psicológicos, en el habla y la dentición, entre otros. El objetivo de este reporte de caso es informar el manejo y los efectos de la terapéutica ortopédica temprana en un paciente infante con labio y paladar fisurado unilateral, el examen clínico reveló cicatriz de la hendidura labio alveolo palatina con presencia de fístula nasoalveolar en el lado izquierdo, maloclusión clase III esqueletal y un resalte negativo. En la terapéutica se empleó un disyuntor Hyrax y máscara facial de Petit con el fin de la expansión rápida del maxilar superior adicionalmente la protracción del maxilar. Después de un año seis meses, se evidenció la expansión transversal del maxilar superior y el adelantamiento del maxilar superior obteniendo resultados clínicos favorables gracias a la cooperación del paciente y sus progenitores.
