Timectomía en miastenia gravis seronegativa: reporte de caso

Abstract

Myasthenia Gravis (MG) is a predominant autoimmune disease in women between the ages of 20-40 with a fluctuating pattern of muscle weakness and the main antibody involved is the acetylcholine receptor. Thymectomy is the principal treatment for patients with thymoma and acetylcholine-positive patients who are refractoriness to pharmacology treatment. Between 10-20% of patients with MG are seronegative, who have shown a better response to drug treatment than those patients with acetylcholine positive MG, however, rescue therapies can be used for those who present refractoriness to treatment. It presents the clinical case of a 57-year-old woman, with seronegative non-thymomatous MG, received immunosuppressive treatment for 2 years with a poor clinical response, presenting limitation of activities of daily living. In this case, the pacient received uniportal video-assisted thoracoscopy thymectomy with the finding of a complete thymus, which was fully extracted. In the immediate post-surgical period, the patient presented an excellent clinical response, that is maintain in more than 2 years of follow-up.
