Tratamiento de maloclusión clase III en paciente pediátrico: reporte de caso.

dc.descriptionLa maloclusión Clase III se caracteriza por presentar una retrusión maxilar, un adelantamiento de la mandíbula o una mezcla de los dos. Dichos pacientes pueden presentar alteraciones en el plano transversal, sagital y vertical en las estructuras esqueletales y dentoalveolares; por ello, se la considera una alteración con etiología multifactorial. Objetivo: reportar el tratamiento de maloclusión Clase III en un paciente pediátrico a través del disyuntor tipo Hyrax conjuntamente con máscara facial. Se reporta el caso de un paciente masculino cursando la primera década de vida, acude a consulta odontológica en la Universidad Católica de Cuenca. En el análisis clínico intraoral presentó dentición mixta primera fase, en el plano sagital mostró relación molar Clase III derecha y Clase I izquierda. El análisis cefalométrico mostró un patrón esqueletal Clase III por retrusión maxilar. Se realizó expansión rápida del maxilar durante 8 días y se logró aproximadamente una expansión de 5mm, posteriormente se indicó el uso de la máscara de Petit con elásticos (16 horas/día) por 6 meses y se obtuvo un adelantamiento del maxilar. Conclusiones: La disyunción rápida del maxilar conjuntamente con la máscara facial fue un tratamiento óptimo con diferentes resultados esqueléticos y dentales, los cuales fueron favorables para este paciente en particular, al observarse cambios positivos y estables en la oclusión. dc.description.abstractClass III malocclusion is characterized by maxillary retrusion, mandibular protrusion, or a mixture of both. These patients may present alterations in the transverse, sagittal, and vertical planes of the skeletal and dentoalveolar structures; therefore, it is considered a disorder with a multifactorial etiology. Objective: To report the treatment of a Class III malocclusion in a pediatric patient using the Hyrax type disjunctor and a face mask. The case of a male patient in his first decade of life who came to the Catholic University of Cuenca for a dental consultation has been reported. In the intraoral clinical analysis, he presented a mixed dentition of the first phase; in the sagittal plane, he showed a class III right and Class I left molar relationship. The cephalometric analysis showed a Class III skeletal pattern due to maxillary retrusion. The rapid maxillary expansion was performed for eight days, and an expansion of approximately 5 mm was achieved; then, the Petit mask with elastics (16 hours/day) was indicated for six months, and maxillary advancement was obtained. Conclusions: Rapid maxillary disjunction combined with the facial mask was an optimal treatment with different skeletal and dental results that were favorable for this particular patient, as positive and stable changes in the occlusion were observedes_ES
