Ítem Acceso Abierto Efectividad de la miotomía endoscópica peroral en el tratamiento de la acalasia(Universidad Católica de Cuenca., 2024) Guazhambo Pérez, David Alexander; Ruilova Mocha, Jonnathan Stalin; Aldás Erazo , Manuel Rafael; 0104903885; 0106105828Introduction: Achalasia is a rare esophageal disorder with severe consequences. Its increasing incidence and prevalence highlight the need to understand its treatment better. Peroral Endoscopic Myotomy (POEM) offers a minimally invasive and effective therapeutic alternative. Objective: To evaluate the efficacy and safety of POEM in treating achalasia, considering its contribution to managing this disease. Methodology: Adopting the PRISMA protocol, specific databases were searched using MeSH/DeCS terms. Studies published from 2015 to 2023 examining the effectiveness and safety of POEM in patients with achalasia were included. Strict eligibility criteria were applied, and a rigorous screening and data collection process was performed. Results: Initially, 168 relevant studies were identified, of which 12 met the inclusion criteria. These studies revealed significant symptom improvement in patients treated with POEM, demonstrated by a decrease in postoperative Eckardt scores. Furthermore, it demonstrated comparable or superior efficacy to other treatment modalities, such as laparoscopic Heller myotomy. Regarding safety, it showed a low incidence of intraoperative and postoperative complications, which were mild and manageable. Conclusions: POEM is a promising option for achalasia, providing significant symptomatic improvements with an acceptable safety profile. Its efficacy comparable to standard surgical options suggests its utility as an initial treatment or alternative in selected patients. Further research is needed to evaluate its durability and long-term effectiveness compared to other therapeutic options.Ítem Acceso Abierto Eficacia de la manga gástrica quirúrgica en comparación con la manga endoscópica para el manejo de la obesidad en adultos. Revisión sistemática(Universidad Católica de Cuenca., 2025) Campoverde Rodríguez, Karen Jhuliana; Sanjinés Rodríguez, María José; Aldás Erazo , Manuel Rafael; 1105893828; 1105335085Objective: To determine the efficacy of gastric sleeve surgery in the obese adult population compared with endoscopic sleeve surgery, with positive results in managing obesity. Methodology: The following inclusion criteria were considered: without language restriction, with an impact factor Q1 to Q4 according to the journals in the different quartiles, with information from the last five years, meta-analysis, clinical trials, and systematic reviews. The exclusion criteria were literature reviews such as case reports, letters to the editor, thesis, incomplete articles, or articles with high bias levels. Sources of information were searched in PubMed, Scopus, Web of Science, UpToDate, Taylor & Francis, ProQuest, SpringerLink, and The Lancet. Finally, the bias was evaluated employing the Newcastle-Ottawa scale. Results: After applying the bias scale, 32 articles were obtained, which were arranged in a chart. The essential elements were extracted from each one, considering the subject proposed within the research problem. Conclusion: The choice between gastric sleeve and endoscopic sleeve surgery is related to the patient's characteristics and needs, considering the risks and benefits of each technique in the individual case.
