The worldwide concern to reduce obesity has led to the development of a number of treatments, from hygienic-dietary measures to a series of drugs with limited or even null results depending on the individual characteristics of each patient. Bariatric surgery is nowadays considered as the most effective therapeutic alternative, being the Roux-en- Y gastric bypass (RYGB) the surgical intervention that has shown the greatest benefit, not only regarding weight loss, but also an important metabolic control in the 3-year follow-up. The aim of the present review is to determine the long-term metabolic impact of BGYR and to demonstrate whether the benefit is maintained over time, or whether the implementation of new therapeutic alternatives is necessary; to this end, an electronic search was carried out in databases such as Pubmed (www.pubmed.com), Scopus (www.scopus.com), Cochrane (www. cochranelibrary.com/es/), Web of Science (www.webofscience.com). A total of 24 articles were obtained, with a follow-up between 3 and 12 years in which a remission of diabetes mellitus, hypertension, dyslipidemia and metabolic syndrome is evidenced, in addition to the consequent weight loss. Health habits will be essential to help maintain the benefits of BGYR. Therefore, it is concluded that BGYR is an effective technique that maintains weight loss in the long term.
