A smile with more than 3 mm of exposed gingiva is called a gingival smile. Treatment should be planned according to its etiology; gingivectomy and botulinum toxin has been the most used therapeutic methods. General objective: To determine the effectiveness of botulinum toxin versus gingivectomy in patients with a gummy smile. Methodology: The present review was conducted under MESH terms, taking the available evidence, in Spanish, English, and Portuguese, in the digital databases of Scopus, Web of Science, Redalyc, SciELO, and PubMed, and 42 articles were obtained. Results: Gummy smile is more prevalent in women; 312 patients were treated with botulinum toxin, from which 97.9 % expressed satisfactory results. On the other hand, 210 patients chose the gingivectomy procedure, from which 84.1% expressed satisfaction with the results, in addition to the fact that this type of surgery required more care in the postoperative period. Conclusions: For adequate treatment, it is necessary to perform a correct diagnosis to have a good enough result for the patient. The use of botulinum toxin, unlike gingivectomy, is a better option because it is less aggressive, reversible, and atraumatic.
