Currently, there are several methods for diagnosing appendicitis, such as abdominal ultrasound, which is a reliable and non-invasive test. The Alvarado scale, also uses a score to evaluate the patient and provide a rapid evolution. Objective: To determine the efficacy of abdominal ultrasound compared with the Alvarado scale as a diagnostic tool for acute appendicitis in adults. Observations: The nature of the study is a literature review, and data collection was performed using open-access scientific databases such as Scopus, SciELO, PubMed, Springer, and ScienceDirect, published from 2017 to 2022; the search will be performed using descriptors in health sciences, along with Boolean operators such as ''AND,'' ''NOT,'' ''OR.'' Conclusions: The Alvarado scale has shown higher acceptance in this literature review because it has high sensitivity and specificity in diagnosing of acute appendicitis, while abdominal ultrasound received lower scores because it is operator dependent.
