According to Operation Smile Foundation, a child with cleft lip and palate is born every 3 minutes in the world. In Ecuador, at the Multidisciplinary Cleft Lip and Palate Clinic of the Catholic University of Cuenca, it was found that males are more prone to suffer from this anomaly, with a higher prevalence of unilateral cleft lip on the left side. This case report describes the orthopedic treatment of an eight-year-old boy with sequelae of a unilateral non-syndromic cleft lip and palate with a class III malocclusion, left posterior crossbite, retrobite, and retrusion of the upper incisors. The Hyrax appliance was placed to transversely augment the maxilla and correct the posterior crossbite, then the face mask was placed to protrude the maxilla and also achieve the pro-inclination and protrusion of the maxillary incisors. This treatment was continued for eleven months, and clinically significant dental-skeletal changes were evidenced.
