The general objective was to report a case of tuberculous mastitis in the Hospital José Carrasco Arteaga in the city of Cuenca; the design used was observational, and articles from academic journals and original case reports were used for the literature search. A 42-year-old female patient is presented, whose initial reason for consultation was a lesion in the right breast with premenstrual mastalgia, umbilication of the nipple of the affected breast, and white discharge in small amounts. Subsequently, a cytology of the mammary secretion was performed, which showed benign Yokohama II results consistent with mastitis. One month later, a breast ultrasound was performed, where the presence of a nodule in the lower quadrant of the right breast was found; a biopsy was taken where coccoid bacteria were reported, so it was decided to send a sample to infectiology for evaluation, and a microscopy study was performed reporting positivity (+); a PCR test was performed with a positive result, and Mycobacterium tuberculosis growth was evidenced. Therefore, it is a case of extrapulmonary tuberculosis; starting antitubercular treatment with subsequent controls without relapses was recommended.
