It is considered one of the causes of mortality worldwide, being a cardiovascular pathology and a risk factor for suffering from other diseases. Objective: To conduct a systematic review of the factors associated with antihypertensive treatment compliance. . Methodology: A systemic review of the scientific literature of Lilacs, PubMed, ProQuest, Scopus, Web Of Science, SciELO, and Google academic tools was performed to obtain the necessary bibliography; related articles were included from 2017 to the present managing inclusion and exclusion criteria. Results: Among the main factors for adherence to antihypertensive treatment, it was found that the average level of education, married marital status, female sex, age older than 60 years residing in the urban area, and a high level of knowledge about arterial hypertension influence adherence to treatment. On the other hand, elements such as male sex, younger than 60 years, primary education level, unmarried marital status, low level of knowledge, medication fee, unemployment, and access to health areas influence non-treatment-compliance. Conclusions: Some factors influence non-adherence to treatment, which should be considered to promote intervention.
