The objective focuses on analyzing the efficacy and safety of endarterectomy vs endovascular graft in patients with stenosis of the internal carotid artery. Development: A literature review was carried out using key terms MeSH and DeCS in relevant medical databases such as PubMed, Scopus, Web of Science and Elsevier. Search strategies will be designed to identify primary studies and systematic reviews that address the comparison of both techniques, evaluating their safety and effectiveness. The results were presented clearly and concisely using tables. Conclusions: This review highlights that carotid endarterectomy continues to be an effective and safe treatment compared to endovascular grafting. CAS is less invasive and may be preferable in patients at high surgical risk or in those who have complex carotid anatomies. CEA showed better perioperative safety if performed at an optimal time, in early intervention. Selection of intervention should be based on a careful assessment of anatomy, general health status, and comorbidities.
