- ÍtemAcceso AbiertoTratamiento de la hipertensión arterial en mujeres gestantes con enfermedad renal crónica(Universidad Católica de Cuenca., 2023) Pesantez Naula, Paula Samantha; Bueno Castro, Andrés Santiago; 0302611538Background: Chronic kidney disease (CKD) has a global prevalence of 13.4% (1), affecting up to 6% of women of childbearing age and about 3% of pregnant women (2). Arterial hypertension affects between 22% and 54% (3), with a higher frequency in low and middle-income countries (32%-90%) (4,5). Objective: To identify the appropriate arterial hypertension treatment in pregnant women with chronic kidney disease. Methodology: A literature review of seven studies (three meta-analyses and systematic reviews, three clinical trials, and one retrospective study) was conducted, collecting and selecting studies from different free access databases such as Medline, Epistemonikos, Lilacs, TripDataBase, and Cochrane. Results: The incidence of severe hypertension was significantly lower when nifedipine and methyldopa were administered. Still, nifedipine outperformed the other drugs because of its relative safety and showed no difference in the risk of hypotension, maternal and fetal outcomes, and adverse effects. They are essential in low- and middle-income countries, where access and drug of choice may be limited. In addition, the use of atenolol/labetalol in pregnant women with pre-existing hypertension is associated with an increased risk of small-for-gestational-age newborns. Conclusions: The incidence of severe hypertension was significantly lower when nifedipine and methyldopa were administered. Therefore, they were considering oral nifedipine as the first-line antihypertensive agent.
