Hemorrhagic CVA can be divided into parenchymal and subarachnoid; these represent high severity and mortality. Objectives: To review the diagnostic and therapeutic hemorrhagic stroke aspects according to the currently available evidence. Methodology: This research was conducted based on the PRISMA systematic review checklist by searching for DeCs and MeSH keywords, and Boolean and or or, in the following databases: PubMed, Medline, EBSCO host, ScienceDirect, OvidSP, Scopus, Research Gate, World of Science, Redalyc, in institutional pages such as ILAE, different national and international universities repositories and in the Lancet journals platforms, The New England Journal of Medicine. The articles used for searching information were published in English and Spanish in the last five years; however, some were added beyond this period because they were essential to the subject matter understanding. Conclusions: This review will help clarify this health problem regarding diagnosis and treatment. The diagnostic approach is based on two pillars, the anamnesis, including physical examination and entity characteristics, and complementary examinations. Treatment is very controversial, with several options ranging from antihypertensive management, elevated ICP, hemostasis, antiepileptic therapy, and surgery.
